• shipping & mailing address
• postal/zip code
• description of material required (cutting list if applicable)
• In Ontario - your PST exemption number (if applicable)
• Customers in the USA - Federal Employer Identification Number (EIN) or if this is personal , your Social Security Number. This is required by US Customs in order that they may process your order across the border.
